As of 12/9/19 the NY Serves Upstate network will join the 360 Collaborative Network with Consumer Credit Counseling Services of Rochester (CCCSR) and Catholic Family Center (CFC). This is an important, and progressive, step forward to expand this successful network to include enhanced care for our veterans and their families. In addition, the benefit to the region will be invaluable in transforming the way care is coordinated through local ownership and connected service navigation. As part of the 360 Collaborative Network, we become a unified force dedicated to a shared goal: revolutionizing health and human service delivery for everyone in our community, now including our military-connected clients . While providers will no longer have the ability to refer directly via the Unite Us platform, the IVMF will continue to host the above assistance request form as a resource for any veteran, service member, or military family member in need of services that fall outside of your scope of practice. Therefore, your clients can continue to make requests into the Network.
